Monday - Friday: 9:00am to 5:00pm

Appointments

To schedule an appointment at Lone Peak General Surgery please call, 801.566.3062 during regular office hours.  Please bring your health insurance card to your initial visit, as well as subsequent visits. In addition, please bring:

• a list of medications you are taking
• a list of known drug allergies
• relevant immunization or medical records
• referral from your primary care physician

We make every effort to stay on schedule, although emergencies arise. If we are seriously delayed, we will attempt to notify you beforehand. As a courtesy to other patients and staff, please call the office as soon as possible (preferably 24 hours in advance) if you are unable to keep your appointment or are going to be late.

Before your appointment, you may wish to write down a list of questions you want to ask. Also, please make a list or bring the medications you are taking, both prescription and non-prescription.

Cancellations

We call to confirm your appointment one business day prior to your planned visit. In the event that you must reschedule an appointment, please notify our office at least 24 hours in advance to avoid a cancellation fee.
